Pressing either Fn + F12, or VO + the Equals/Plus key, increases the system
volume.
This is not the same as increasing the volume of VoiceOver, relative to the
system volume.
To adjust the volume of VoiceOver, relative to the system volume, you need to
access the Speech Attributes rotor.

Of course, this makes it quite difficult to go into voiceover configuration to
raise the volume. Can anybody tell me the key sequence to use to increase
