Problem solved, it turns out that VO was set to speak what was under the mouse,
and this was causing the chatter.
I cleared that setting and the problem seems to have gone away.

Try VO-f1-f1 (Application Chooser) and/or VO-f2-f2 (Window Chooser) to see if
there's an app dialog causing this. Sometimes, when something like Adobe
Reader wants to update itself, this sort of behaviour occurs.

Since upgrading to Mavericks last night, VO keeps saying "Macintosh HD Volume
