I am glad to help. Nisus was one of my first program for my mac, when I got it
4 years ago.
If you have the original document. You could try to sent it to :
supp...@nisus.com. It could be interesting to figure out why the problem exist,
it could be a mistake.

Hi Annie, sadly, using the context menu produced the same results, IE the
heading levels just don't appear. They're not greyed out, they just aren't
there at all.
What I've done though is taken the text I wrote in text edit, copied it to the
clipboard, and pasted it all into a new document in N

Select your heading, and press vo+shift+m, find add to toc, and select the
level in that menu.
You can also choose paragraph style in the menu, and choose your heading level
there.
