I'd like to use the DocBook Book document class, but it is listed as
unavailable. I have all the DocBook tools installed (as that's what
I've used the last several years). What do I need to do to make the
classes available to LyX?

I'm using the book document class. It seems that chapters are the only
high level entity to be included into a ToC, and it seems Chapters and
Sections are the high level classes for the document. This seems -
well, not odd so much as plain wrong. Most books I have would include
things like

When I go the the Tools/TeX Information menu, it shows in the Styles,
and will show me the contents of the file. However, when I process my
ducument, I get two errors, both 'Unspecified control sequence', the
first at \m
I've just install TexLive & LyX on a new OS install; when I open any file in
LyX (the help files) I get errors that class files are not found. They are
indeed on my machine. Looking at the debug messages, I see:
sh: kpsewhich: command not found
However, the file is on my path:
