I guess the correct way (whatever that means) would be to redefine the
\maketitle (or [EMAIL PROTECTED], as I've seem sometimes -- don't know the
difference) macro. It's quite easy if you know some LaTeX (from head,
might contain typos):

I'm using a lyx template I found at Durham university for my thesis.
It has a title page similar to what you want and can easily be modified
to fit your requirements. Take a look at

N.B. Using linux it works fine but my windows colleagues had to install
a few extr
For my PhD thesis, I did the cover page in gimp, and
included it as an eps file, scaled to 95% of the page
width. That way I had it included in the resulting
pdf file. It was much easier to get the layout,
rather than doing it in latex, or lyx.
