The error box appears right at the top of the document, before the
title. I have tried what you advise. No luck!

vanilla' preamble which only includes
\usepackage{SIunits}

It's standard in Lyx that you do not add the .bib suffix. See the User
Guide.

I have copied exam.cls (from CTAN) to my /usr/share/texmf/tex/latex/lyx
directory, run texhash and Lyx reconfigure, restart and the new class
does not appear in the Documents:Class field.
What next? I'm not sure how to set up a new .sty or .layout file

I have copied exam.cls to the /usr/share/texmf/tex/latex/lyx directory,
run texhash, reconfigured and restarted Lyx. I cannot see a reference to
this class in the documents>layout>class field.
There was no .sty or .layout file in the package. How do I fix this?

Does anybody know of a newsletter layout/class that will support things
like: vol#, no#; banner headlines; date in the header; sidebars; boxed
text and balanced columns. Since the old Mac died I've been using MSWord
but would like to keep all my docs in Linux.

I have included the line
\fax_command "fax send $$Phone $$FName"
in .lyx/lyxrc but nothing seems to happen. I cannot find a menu item to
fax files.
Using Lyx 1.1.6fix1 - have not been able to upgrade to fix 3.
