I think I solved my problem. It seem that the errors occur only if
I use Lyx style drop-down list for title, author, address.
So, I used Latex command for those style and no errors at all.

I've noticed that whilst running LyX and previewing a .pdf file on my (default)
viewer evince, that LyX will create zombie processes of evince. This happens
every tiem the file is previewed. It will also list a process as being a zombie
even though that instance of evince is still ope

I am going to suggest (again) the package idxlayout. I am worried
about weird interaction between packages as much as you, but I found
out a good package is often safer that adding several lines of latex
