I am using Book-memoir and all the endnotes appear in the form of footnotes;
that is, each starts with a superscript number which is indented and
followed immediately by the endnote text.
If there are subsequent lines for that footnote, they are not indented.

My bad. The bug is _not_ fixed in 1.6.7; you can modify the path but you cannot
delete it from the preferences menu. (You can manually edit the configuration
file to get rid of it.)

1. Put the text you want aligned with the table in a parbox (Insert > Box,
right click on the box's handle, change type to parbox -- although a minipage
probably works just as well.

While using *memoir*, this will work at stripping the first page of all
chapters of any page number, heading or footer: just put
\*aliaspagestyle*{chapter}{empty}
into the preamble. Alternatively, create a new page style that looks like
