I suspect this is a problem with the font scaling, because when you zoom in
and out in Lyx this goes away!

1) A couple of menus on the math panel are blank and the symbols they
contain don't show up in lyx, but do show up in the dvi.
2) Same thing happens with superscripts, but not subscripts.
