I had a look (again) at the macro stuff. It's more complicated than I
would wish and it looks as if we basically have a choice between "to
what LaTeX does" and "everything is an inset", but we can't get both.
Patches for both versions attached. diff1 is 'all inset' aka 'what we
have now' and diff
