Here is the patch as it stands. I'd be interested in comments.
One issue is that I've made cells_ mutable, which seems dodgy.
Another is that the name & numArgs cells in macrotemplate are
just normal cells. I think they should be something that only
allows macroargs

I noticed this had been done. It seems to have fixed
the bug. I've built & tested on XP & fedora 3. I had noticed
there was previously a big delay when creating the first
math insert in a blank document - this has been fixed also.

A colleague of mine have installed Uwe full installer. It seems that the
default miktex installation doesn't contain htlatex. Plus, even if you
install it manually with Miktex, lyx doesn't check for htlatex on a
reconfigure, only tth and hevea AFAIR.

Sounds like you didn't ./configure with "--enable-maintainer-mode" - try
running configure again.

I downloaded and compiled the 1.4.0pre3 version of LyX. When I opened a
lyx file, the math macros are loaded empty; with no name nor content. If
I save the document, the lyx file will also have empty math macros. A
work around is to close the file without saving and reload it.
