"Long" fermata above two notes

Hello, I've been trying to typeset a piece of music that has a "Long" fermata above two notes. It is used sometimes to indicate the place where the soloist should play the cadenza in a concerto. http://old.nabble.com/file/p27480522/Fermata11.bmp Does anyone have any hints on how to typeset this?
