Found the issue.
Something, somewhere messed up the Ghostscript permissions.
I ran:
sudo chown -R `whoami` /usr/local/share/doc/ghostscript
brew link --overwrite ghostscript
Which fixed the issue on the following invocation of lilypond.

reinstalled frescobaldi. i've tried running LY direct from the command line
