While looking for information in the NR I came across a duplicate index
entry. A bit of copy-and-paste and text manipulation and I find that there
are 65 - I've got the full list but a simple example is acciaccatura. Is
there any reason for this?

Official tests are fine. Some more instances of filename changing
generating logfile discrepancies, and the odd errors in logfiles from
"Festival" output, but nothing else.

Having a rethink on this section, after Graham's comments.
Currently trying to simplify some of the examples and explanations. Also
need to include the \markup function \auto-footnote as well. I'll close
the Reitveld and open a new one when I'm done (hopefully in the next day
or so).
