I had the same problem with the image generation using the Linux distribution
Suse 9.3 (which includes the latest version of most tools).
The trick of modifying in the file
/usr/share/texmf/dvips/misc/alt-rule.pro
to rename /QV to /QV_Hide fixes the problem.
