It does get created automatically IF one includes a table of
contents.

Just installed
latex2html-init99.1 release
on a machine running redhat 5.2 GNU/Linux 2.0.36.
Perl is 5.005_02 compiled without threads.
I turned the frames on and it looks very nice.
Just two problems:
