I'm really sorry for not answering earlier, i was VERY bussy with this and
other events we had here...
At that time i managed to figure what happened and solve it because i didn't
lost all that data, BUT now it's happening again...
The thing it's with duplicates of issue_id in tables
Hi Muirunyeri
You MUST fix the issue with the duplicated records due to auto_increment
issue before anything.
Follow the instructions on the about/wiki page to fix it.
Once fix that issue, check if your error continues.

I hope it solves this, so i may try it now
The good thing of this method is that i can manually update some ids to not
lose those duplicated records :D
(Already tested with at least 1)

Hey Tomas,
Good tip. I did as you suggested, and there were a number of additional lines
in the section of the koha-conf.xml file, including a couple of lines
about the plack config. I modified the config section of my main koha-conf.xml
to match, changing the file paths and other references
