We did an advanced search for title "David" and author "David Shannon" (without
quotes) and not phrase searches and ended up with 79 results. Of these 79 it
looks like only 12 have David in the title. Is the 245 $c field also being
searched in the title index?

Quite a while back we had a problem with duplicate biblios appearing for
newly added catalogue records.
But this time it's only one. It is an exact duplicate of a record that had

On further investigation, this seems to be a problem with the link parsing.
I'm not sure how I can get to the bad record, because Koha keeps bringing
up the good record. There appears to be some very small differences, as if
the record was saved in mid edit, and saved again, with the same timestamp

Hm, I haven't tested, but check your setting for the SingleBranchMode
system preference in administration - it will hide hints to the branch
name and search options for limiting by branch in the OPAC.
