[Koha] Weird textboxes in koha 3.10.4

I have 3.10.4 installed in debian 6.x. I'm getting this weird textboxes When I get to the detail of the item, I get three search textboxes instead of one. Hmm... Anybody experienced this with koha 3.10.4 Thanks and more power
