Koha doesn't keep a record of logins or login attempts. If
SessionStorage is set to 'MySQL database' looking at the sessions table
might give you a clue.

print slip that wraps around a book. If so, this could probably be done
with the HOLD_SLIP and some CSS.

Wow, that's a really interesting question -- I think you're right, if the
patron is going to get a notice, the'll get debarred if one of their
overdues triggers the notice, regardless of their relationship to the
branch.

Koha will print a hold slip when an item is checked in that can fill a
hold. The slip and its contents can be customized by library using HTML
and CSS. So I think the answer is yes :)
