I just upgraded my Acer Aspire V3-771G with Nvidia GeForce GT630M to
16.04 from 15.10. What I found was the following:

I modified the LOG variable in /sbin/prime-offload to /tmp/prime-
offload.log as well as adding xrandr -q >> $LOG 2>&1 as per darkxst
suggestion and only get one line logged: Screen 0: minimum 8 x 8,
current 8 x 8, maximum 16384 x 16384

I just want to share a quick observation related to this bug. I've
noticed that when there is a kernel update and doing a soft-reboot, this
problem shows up. When this occurs, I do a soft-shutdown and power up
and the problem is resolved.
