You need to check you firmware version:
> sudo hdparm -i /dev/sda | grep FwRev
Model=Samsung SSD 840 EVO 250GB, FwRev=EXT0DB6Q, SerialNo=x
If you have the latest rev EXT0DB6Q, then you need the fixed kernel
mentioned above.

Uwe: No, Windows will not update the SSD firmware by itself. Samsung has
a windows application for updates, but it has to be downloaded from
Samsung and executed.

The fix is in kernel 4.0.5 and I can confirm it fixes the problem. Been
running the latest firmware on my EVO 840 SSD for a few days now and
everything looks good.
