It seems the VC4 device tree overlay is not applied by the Rpi firmware.
I tried adding dtoverlay=vc4-kms-v3d and adding a corresponding
.../overlays/vc4-kms-v3d.dtbo file but it seems the Rpi bootloader does
not apply the overlay at all.
I merged vc4-kms-v3d.dtbo into bcm27

I just found that patch d894be57ca92c8a8819ab544d550809e8731137b is not
required, since it depends on a52ad514fdf3b8a57ca4322c92d2d8d5c6182485, and
a52ad514fdf3b8a57ca4322c92d2d8d5c6182485 is not in 4.8 tree.
Thus, not adding d8

I've attaches a copy of syslog which records the system booting with the
Thunderbolt attached, then being removed and re-inserted.

No, it's not widely broken. Some things are broken on some hardware, and
it's not all broken in the same way, that's why I asked for logs -- so
we can know what the actual issue is.
I don't have all the hardware in the world, so I can't reproduce these
issues myself.

Joseph, any more kernels for me to test? It's been a while.
Also, I ran across bug #1603476, which reports a failure with kernel
3.13.0-92 that might (or might not) be related to this.
