I'm using 14.04 on an HP ProBook 6460b.
I have the same issue.
It used to dim the laptop screen when the monitor is unplugged. Then
plugging it in again and removing it brought the controls back.

The intel error only causes disconnects, but not all of them are because of
intel error. Often it disconnects by itself, without any other errors.

When the system (Ubuntu 16.04 LTS with Kernel 4.4.0-28) goes into
suspend (hybrid-sleep with systemd), I get sometimes (around every fifth
time) a system freeze. In the other cases, everything works fine before
and after the suspend with the system and DVB-C/T card DVBSky T982
I have also got this bug, but from a fresh install of ubuntu 16.04.
Using an SSD,
