The empty box is a open terminal which is minimized.
The Firefox Icon to the left shouldn't be there because

Here the Icon of the open Firefox ("Logout ...") is missing and the firefox
icon to the left of that is too m

Here you see some strange free space in between the Fast Start Icons.
And there are again the icons for firef
