I haven't been able to reproduce this in a while. If you're right though, that

I think the underlying issue is that pulseaudio/plasma-pa doesn't go back from
the headphones port to the speaker port if the headphones are disconnected. All

Ok, I was able to reproduce it again. To reproduce do the following:
1. Plugin headphones.
2. In the sound KCM, configure to use the speaker port, not the headphones.
