1) the "quota bug" has existed for years, a workaraound is to run quota -f
twice. That may also be the reason why you see
I did some tests...
When I transfer my "problem mailbox" with the XFER command to another host
(with exact the same version & configuration), there are no problems!
When I retransfer the mailbox to the "problem host", the problems are back...
Any ideas where problem could be?

There are difference between "quota -f" and "quota -f user.xyz":
