[Groff] mom fix

A couple of bugs affecting collating, tables of contents and lists crept into mom during the 1.3 cycle. I've got them fixed (I hope), and committed the fixes (version 1.3-e_3) to the repository and posted a mom tarball on her homepage. If anyone's experienced problems, I suggest upgrading. -- P
