All the different cases in output_loc_list could use brief comments.
Let's keep loc_list_idx, output_loclists_offsets, and
assign_location_list_indexes together in the file.
OK with those changes.

The patch doesn't handle -gdwarf-5 -gsplit-dwarf on targets where assembler
doesn't support .uleb1
