The problem is probably that the image is outside of the tkregister2 FOV -- I noticed that there is a large shift from magnet isocenter in your image (about 30 cm). tkregister2 centers the FOV at isocenter. I still have not found a work-around for this, but you can use mri_vol2vol --targ
