When I do the same thing on 11.2ish with a 16-byte plaintext file, it uses
aes-256-cbc and produces a 48 byte output.

This is sort of documented in the HISTORY section of openssl-enc(1):

The default digest was changed from MD5 to SHA256 in Openssl 1.1.0.
