As far as i understand is that you want only the userform not the excel
workbook.
If this is the case you need to use VISUAL BASIC for preparing the form not
vba which is inbuilt in any application.

Hi Anil,
Try this,
Go to windows Explorer then
Tools>Folder Options>File Types tab and select XLSX files from the list.
Click on the Advanced button which will open up the Edit File Type dialog
and you should see the Change Icon button on the top.
