I started experiencing this issue after upgrading from 13.04 to 13.10.
Memory used by unity-panel-service goes up after each application open /
close and never seems to drop again. It piles up to hundreds of MB (even
GBs) and eventually uses 100% CPU at which point it must be restarted

I am experiencing this as well. After upgrade I the package appmenu-gtk
and appmenu-gtk3 are no longer installed and when I try to install them,
I receive this error:

Actually I just installed the Numix Gtk theme - and now it works. Even
with 1.5 vs 1.0 pixel scaling between the monitors.
Appears to be a scaling issue with the images used for
close/maximize/minimize in the default ubuntu theme.

I ran into this after an update from 16.04. I had a custom panel as well
but I found a way to show the notification panel again without resetting
the dconf configuration. I had three indicator-application.desktop files
(and I IIRC likewise as many probably competing running instances of
it):

Hm, actually I forgot about this and at least for 14.04 (Trusty) this is fixed
by now. Might have been some intermediate problem during development, too. I
had cases where for some unknown reasons parts of the touch image binaries
leaked into desktop.

I've seen the same behaviour after an upgrade from 12.04 to 14.04. Login was
still possible but the screen did not unlock. I had this message in
/var/log/auth.log:
hostname unix_chkpwd[26503]: could not obtain user info (username)
For me this was caused by wrong file permissions on /etc/shadow.
I have the same problem, though I don't have any unix_chkpwd errors as
it's not installed on my systems.
I was able to solve it by replacing:
1) libpam-ldap by libpam-ldapd
2) libnss-ldap by libnss-ldapd
3) nscd by nslcd
I think it is related to my use of self signed certificates that libpam-
ld
Same problem here on the BQ Aquaris E5.
