I have a workaround for this, use Gnome instead of Unity.
It took me a while to realize/figure out that it's pretty painless to
just run a different window manager on Ubuntu. I did look at maybe
switching to a different lock screen but there was no straightforward
way to do that.

I've also seen this problem where the input field is missing on one
monitor, but not on the other.
