I 'm experiencing this with qtcreator from qt sdk online installer.
Seems qtcreator disappears from unity app switcher and launcher after
suspend, with the qtcreator process running.

Had the same problem (System powered off, but hardware did not actually turn
off) on a dell vostro V131.
Problem persisted on installed Ubuntu and Windows 7 as well as Ubuntu and
Fedora live systems (from USB)

Menu in audacity can't
I took a stab at this one...
Reverted the AppLauncherIcon to the earlier state that will grab focus
if it gets the scroll event, and changed Launcher itself to check its
new setting activate_on_scroll to decide if the event should even be
sent. New option is exposed at the bottom of the CCSM Unity
And the way that the window should look.
This issue does not happen on Ubuntu 12.04 64bit

This affects installers and possibly other applications which run with
wine. I'll add screen-shots to better illustrate the issue.
The workaround is to configure wine not to let the window manager
decorate windows.
