Does the "mbox_lazy_writes" settings only affects the mbox backed? In
our case we use mdbox, thus disabling this setting would not have any
effect, right?

By far the best and easiest solution would be a flag to auto-create the
special folders ONLY on first login.
Is there any chance to get this feature?
