I looked at the questions and also the spreadsheet. I don’t see any issues.
When you promote this survey make sure users know you are sending them to
Google (before they click). Bonus points for adding the privacy terms.
As long as this form is community driven and optional, it is fine.
I had a look at some options. All of them are either paid or very
limited in the free version. I would love to use Limesurvey just like
the ASF but getting the paid version is probably too much effort for our
small survey.

FYI I think it should be no problem to find a free or affordable survey

Cordova project. The survey would be anonymous, but we could include an
optional field asking for the email address. I would not include that
