This bug affects the latest version of Ubuntu as well. This is an important
issue for me since I need to print a custom size. Does anyone know a manual
work-around until this issue is resolved?

I am having the same issue where custom fields are not displayed.
Trying to enter a custom PIN for print jobs does not display the input
box required to type in the PIN.
$ lsb_release -a
LSB Version:
core-2.0-amd64:core-2.0-noarch:core-3.0-amd64:core-3.0-noarch:core-3.1-amd64:core-3.1-noarch:
