I have attached screenshots for Muine and Totem, when I get home later today I
will post an epiphany screenshot and try to check which are the offending icons.
I'm using the tangerine icon theme.

This is definitely related to the new Human gtk theme, I tried it with
many icon themes and all of them show the same glitches, but only with
Human controls, it doesn't happen with Clearlooks or Glider for example.
