Looks like it's the same bug. Most users i encountered who have the same
issue are on Intel UHD graphic cards like me.

So far it only affects Ubuntu Unity 21.04. Main Ubuntu 21.04 doesn't
seem to have this issue and System monitor opens without any problem.

This is the error i get when trying to run the system monitor from a
terminal:

The Extensions app isn't installed by default but is pulled in when installing
Gnome Tweak Tool.
What lotuspsychje means is that he installed Gnome Tweaks to get the Extensions
app.

build as of 26 November 2012.
