Gnome keyboard layout options (like remapping Caps Lock) are not reapplied
after reboot
Unfortunately bug wasn't fixed yet, at least after updating to Hardy today.
Interesting thing, that after disabling "Enable Automatic Login" in GDM
configuration problem was fixed.
It's possible that GDM performs some initialization related to
gnome-settings-daemon when not started in Autologin m

I *think* I've had to turn on the "ignore duplicate keypresses" setting

I meant gstreamer plugins, not rhythmbox plugins.
Run the following commands in terminal ( Terminal Emulator )
1. sudo bash ( enter your user password when prompted )
2. apt-get install gstreamer1.0-plugins-ugly
Now extract should work fine.

There is nothing to show up in "Tools" ( menu is empty ), when rhythmbox
is switched to fullscreen.
"Tools -> Plugins" is directly accessible as "Plugins" in the main menu.
If you enable "Python Console" plugin, the "Tools" menu will have that
entry though.
