I did a "apt-get source empathy" in Karmic to quickly check this out.
In file empathy-status-icon.c, a pixbuf is loaded with function
"empathy_misc_get_pixbuf_for_notification", which can return a NULL. If
the pixbuf is NULL, then the notification bubble never receives an icon.
So I went hunting
One thing that might work is replacing, in function
"empathy_misc_get_pixbuf_for_notification" , the call for
"empathy_pixbuf_from_icon_name_sized (icon_name, size)" by a call for
"pixbuf_avatar = empathy_pixbuf_from_icon_name_sized ("stock_person",
size)".

unwanted delay. I suggest adding to that a momentum-based approach that
recognizes whether the mouse was placed or thrown by analysing its speed

Nicolò, the Ubuntu version of Empathy shows the old status icon if the
option "use message indicators" is unchecked. Does everything work
properly when it is unchecked?
Maybe it could be possible to "eat" the notifications only when this
option is unchecked, but I don't know how hard would this be
But I think this bug is only downstream, as this eating does not occur
with a vanilla Empathy. The solution probably relies on fixing the
Ubuntu patch that adds support for the Messaging Menu and disables the
status icon.

enough, when I insert a real CD the phantom audio CD is replaced by it.
