I can confirm i have the same issue (22.04). I have Tweaks -> Windows -> Focus
on Hover set. When i close a window with GUI (click X) the window underneath
(the X button) gets brought over the top of all others. I note that using Xorg
or X11 is a workaround but for some reason gdm3 is breaking

though, it looks like only some apps using the indicator are triggering

Here's a workaround that may help. Close your settings app then run
these commands in your terminal
gsettings reset org.gnome.desktop.interface gtk-theme
gsettings reset org.gnome.desktop.interface icon-theme
Then open the Settings app and set your preferred theme.
