I have this same issue. I generally only set the "Run A Terminal"
keyboard shortcut so I dont know if this affects any of the other
shortcuts, but when I set the "Run A Terminal" shortcut to
"Ctrl+Shift+Q" it will get unset and revert back to disabled on a reboot

I don't think I have the exact document(s) that caused the crash anymore
since the dvi is just an intermediate file but I can attach a later
