As usual, you start up with one single box visible asking clearly for Username.
After entering your username, that box stops being an edit box (instead just

A part of the issue is #23898, Jeff is supposed to update the theme to have
username and password separated too ... Jeff, is that update still on your list?

I also think that a more visible feedback would be better. You barely notice
the difference between the username and password entry, only when you know what
to look for or pay a little attention. I suspect most people don't do that.
