I have uninstalled all extensions and the problem still occurs.
Interestingly it is fully repeatable on one system but intermittent on
the other...
Apport log uploaded, thanks.

Public bug reported:
Using Vanilla Gnome session, with extensions turned off, the dash icons
on the left of the screen do not display properly.
The topmost icon is always missing (with the exception below) and
replaced by a small empty square (which can still be clicked to open the
application).
This is also affecting the version in Zesty.

I can confirm this still happens with gnome-shell 3.24.1-0ubuntu1.
It appears that you can stop this from happening by blocking gnome-

I should add - for me I get "nautilus crashed with signal 5 in
_XReply()" - not in _XEventsQueued().
If nautilus is run from the console the following is output:
(nautilus:6095): Gdk-ERROR **: The program 'nautilus' received an X Window
System error.
This probably reflects a bug in the program.
I have had this issue in both 14.04 and 16.04 when changing wallpaper.
There is a very high (50%?) chance that changing the wallpaper will
crash nautilus.
Since it might be X related, I'll add that I'm using the open source
"radeon" driver not a proprietary graphics driver.

I have this issue in 16.04 and had it in 14.04 as well. It is very easy
to reproduce; connect HDMI monitor, nautilus crashes. It happens both on
connecting and disconnecting.
I am using the open source Radeon graphics driver if that helps.
