Do you have the focus prevention setting enabled?
For example I have a similar issue with the Run dialog (commonly alt+f2
on kde). I press alt+f2 and it remains in background. This is a very old
issue, that they introduced in the 0.3.x branch and they never fixed.

Actually the frames of those windows are not updated at all. For
instance a Konqueror window with dead frame will not change its title
bar if another page is visited.

Some windows do not change their frames to indicate that they are
active when I switch to them with Alt-TAB or the mouse. They respond
normally to input, so it's only the visual indication that is
missing. The frame remains
