Continuing my attempts using my slimmed-down example ( https://github.com/mmguero/deblive_blank) I am now getting the BIOS installer to use my preseed correctly, and get through the "copying data to disk" phase. Now debian-installer keeps exiting and being restarted. The error I see is: main-menu:

Hm, after more looking at it I get the feeling the preseed file at /cdrom/install/preseed.cfg isn't being used at all in the BIOS install. It's asking me all sort of questions that I feel like it should be getting from the preseed. -SG
