I would recommend an Ubuntu host and the live-build version 3.a~57
available in Ubuntu repositories. That's how I had success to build an
Ubuntu image without problems.

live-build should not leave an empty fstab behind because it breaks
live-installer (and live-boot/config?) if it does. live-installer will
not create a new fstab

the following two patches will make live-build use LB_APT_HTTP_PROXY in
debootstrap and also to download the d-i udebs. This can save something
between 5 to 10 minutes in download time each build.
Because I only use apt-cacher-ng I did not tested this with any other
proxy installed,

Using the following live-build hook stops the original starpar error.
