Mean take the current bf2.4 linux.bin and the image-1.44 also bf2.4
root.bin, copy booth to the
'/tftpboot/' directory and pass to default PXElinux config the two next
lines:

When I insert or remove the card dmesg show nothing.

Check that there's no function in the BIOS to disable it. Note that
'hot plugging' an external monitor has never worked for me, it needs
to be plugged in at boot time. I've also found that restarting X
(Ctrl-Alt-Backspace) sometimes will kick the X server into
recogniz

Most of the SD-card readers are usb based. Try to use usb-storage
anyway. It would not wonder me, if this also just was a build in usb
sd-card reader.
